SCHEDULE  - I :

2. Admission fee i)   Rs. 20.00 for Hospitals
    ii)  Rs. 10.00 for CHCs/PHCs.
3. Private Wards :  
  1. A- Class Room (Class I Room) Rs. 150/- per day (Class-I Gazetted Officers)
  2. B- Class Room (Class II Room) Rs. 113/- per day (Class-II Gazetted Officers).
  3. C-Class Room (Class III Room) Rs. 75.00 per day (Class-III employees)

SCHEDULE - 2 :

1. Registration fee & Annual Renewal fee for pharmacist/Compounder/FHW/Nurses. Rs. 400.00 (Regd. fee) Rs. 40.00 (Renewal fee)
2. Licence fee for running a pharmacy per licence per annum for : Licence fee for running a pharmacy per Licence for :
  (a) Chemist / Drugist whole seller a) Whole Seller Rs. 400/- per Licence.
  (b) Drug shops under Restricted Licence. b) Chemist and Drugist Rs.200/- per Licence.
3. Renewal fee above a) 200/- per annum.
    b) Rs. 120/- per annum.
4. Late fee above (A&B) Rs. 100/- per month for a,b,&c.
  a) Fee for issuing duplicate Licence for above (A&B) Rs. 200/- for a, b &c.
5. Manufacturing Drug Licence fee (D.L.in form 28) Rs. 2000.00 per Licence + Rs. 1000.00 (first Inspection) and Rs. 600.00 (2nd Inspection).
6. Renewal fee for D.L. in form 28 above. Rs. 1200.00 per annum
7. Late fee for above Rs. 400/- per month or a part thereof in addition to the Inspection fee.
8. Fee for issuing duplicate Licence for above Rs. 1000/-
9. Manufacturing Drug Licence fee (D.L. in form 25) Rs. 1000/- per Licence + Rs. 200/- (1st Inspection) and Rs. 100/- (2nd Inspection).
10. Renewal fee for D.L. in form 25 above. Rs. 800/- per annum.
11. Late fee for above Rs. 400/- per month for a part thereof in addition to the Inspection fee.
12. Fee of issuing duplicate Licence for above D.L. 25 Rs. 200/-
13. Re-packing Drug Licence fee (D.L. in form 24-B-Re-packing of drugs excluding schedule 'X' Drugs). Rs. 400/- per Licence + Rs. 200/- (1st Inspection) and Rs. 100/- (2nd Inspection).
14. Renewal fee for above 24-B. Rs. 200/- per annum.
15. Late fee for above. Rs. 100/- per month or a part thereof in addition to the Inspection fee.
16. Fee for issuing duplicate Licence for 24-B. Rs. 200/-
17. Homeopathic Drugs Manufacturing Licence in form 24-B (Mother tincture) fee. Rs. 400/- per Licence + Rs. 200/- (1st Inspection) and Rs. 100/- (2nd Inspection).
18. Renewal fee for above 25-C Rs. 200/- per annum.
19. Late fee for above Rs. 100/- per month + Rs. 20/- Inspection fee.
20. Fee for Duplicate copy above 25-C. Rs. 100/-
